Working from the mentor approved version of the proposed study drafted and approved by your mentor in Assignment 4, determine the appropriate site and/or instrument permissions needed.
Complete the Permission Plan posted in the course room and this assignment.
Title your submission Permission Plan
Submit your plan along with a copy of your approved Assignment 4 in Microsoft Word format via the dropbox by the deadline identified above.

This submission will be reviewed by your mentor for a determination of its appropriateness as given the DBA Rubric and DBA Template. ONLY after mentor approval of the Permission Plan proceed to obtain the appropriate permissions. Any student, who does not obtain mentor approval of the Permission Plan and obtain the necessary permissions by Week 8, must schedule and participate in an Evaluative Telephone Conference (“ETC”) with the chair and mentor to determine the viability of the student’s progress towards completion of program objectives.
A student who does not obtain mentor approval of the Rationale for Methodology and Design, Operational Definitions of the Study, and the Assumptions and Limitations by Week 4, must schedule and participate in an ETC to determine the viability of the student’s progress towards completion of program objectives. Such students may remain in the present course room, but will be required to repeat Assignments 1 through 4 until mentor approval is obtained. If at any point after week 5 mentor approval is obtained, the student should proceed to the Week 5 assignment.Note: Failure to schedule and participate in the ETC is automatically grounds for a failing grade in this course. (See Mentor Course Room Completion Policy)
Students, who have completed the ETC but have not yet obtained mentor approval, should complete Assignment 1 this week.
